Notice of Privacy Practices
How your health information may be used and shared, and your rights.
Last updated: October 15, 2026
This notice describes how health information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
1. Our Commitment to Your Privacy
VAVA CONSULTING SERVICES LLC is required by law to maintain the privacy and security of your protected health information (“PHI”), to give you this notice of our legal duties and privacy practices, to follow the terms of the notice currently in effect, and to notify you if a breach occurs that may have compromised the privacy or security of your information.
2. Substance Use Disorder Records
If you receive substance use disorder services from us, records of those services may also be protected by federal confidentiality rules (42 CFR Part 2), which can be stricter than HIPAA. In general, we will not disclose information identifying you as receiving substance use disorder services without your written consent, except as those rules allow — for example, in a medical emergency, for certain audits or research, or under a qualifying court order. Records protected by Part 2 will not be used in civil, criminal, administrative, or legislative proceedings against you without your written consent or a court order.
3. How We May Use and Disclose Your Information
We may use and disclose your PHI without your written authorization for:
- Treatment — providing, coordinating, or managing your care, including consulting with other providers involved in your care
- Payment — billing and collecting payment from you, your insurance plan, or another third party
- Health care operations — running our practice, such as quality improvement, training, licensing, and business administration
5. Other Uses and Disclosures Permitted by Law
In limited situations, and subject to applicable state law and 42 CFR Part 2, we may use or disclose your information without your authorization:
- When required by federal, state, or local law
- To report suspected abuse or neglect of a child, elder, or dependent adult
- To prevent or lessen a serious and imminent threat to the health or safety of you or others
- For health oversight activities, such as audits and licensing inspections
- In response to a court order or other lawful process
- For public health activities, as required by law
- For workers’ compensation, as authorized by law
- For specialized government functions, such as national security, as required by law
6. Your Rights
Regarding your health information, you have the right to:
- Inspect and get a copy of your records, in paper or electronic form
- Ask us to correct information you believe is incorrect or incomplete
- Request confidential communications, such as being contacted at a different address or phone number
- Ask us to limit what we use or share; we will agree to not share information with your health plan for services you paid for in full out of pocket
- Receive a list (accounting) of certain disclosures we have made
- Get a paper copy of this notice at any time, even if you agreed to receive it electronically
- Choose someone to act for you, such as a legal guardian or personal representative
- File a complaint if you believe your privacy rights have been violated
