Questions to Ask Your Insurance Provider
Call the member services number on your insurance card and ask:
- Does my plan cover outpatient behavioral health or substance use counseling?
- Do I have out-of-network benefits, and how do they work?
- What is my deductible, and how much of it have I met this year?
- What is my copay or coinsurance per session?
- Is a referral or pre-authorization required?
- Is there a limit on the number of covered sessions per year?
Your Right to a Good Faith Estimate
Under the No Surprises Act, if you are uninsured or choose not to use insurance, you have the right to receive a Good Faith Estimate of the expected cost of your care.
You can ask for a Good Faith Estimate before you schedule a service. If you receive a bill that is at least $400 more than your estimate, you may be able to dispute it. For more information, visit cms.gov/nosurprises.
